Fill
in the blanks with your identification, contact, accommodation request, exam
selections, and payment information. Please set your print margins small
enough to keep the print on one page (if possible), and print the completed
form. Ask one of your parents to sign the form, indicating that you have
permission to be miss your regular classes during the tests. Bring the
form your guidance office,
with your payment.
