Key NorthWest Crew Membership Guidelines
To be an active crew member of the Key NorthWest Parrot Heads, you
need to do the following:
1) Print out and complete this application:

Then, send in the filled out form along with your check for $25.00 for
a single membership or $35.00 for a couple or family membership (checks
made out to Key NorthWest Parrot Heads) to: Key Northwest Parrot Heads,

SW Murray Blvd., #212, Beaverton, Oregon 97008.
2) Attend at least one (1) club meeting (aka phlocking) per year
(preferably LOTS more!). Hey, we're a social club!
3) Participate in charitable activities either with the club, or if
that is not possible, do charitable activities on behalf of the club
and let us know each January and July to make sure that we know what
you are
doing.
4) Help sponsor or organize a club event each year. It can be a
phlocking, charitable activity, help with the newsletter, fund-raising
or
other club activities.
Ophicial Disclaimer: Please note that club dues are not deductible as
a charitable contribution for Federal income tax purposes. No Lifeguard
on duty.
