Subject: Special Medical Needs Form
Date: Monday, July 27, 2009, 9:22 AM

Each county in Florida is required to have shelters designated as “Special Needs” shelters. The purpose of these shelters is to provide a safe location to accommodate individuals who have special medical needs during and after a hurricane, or other disasters, within their county. Because this facilities will have electricity, this is especially important to people who are dependent on an oxygen supply or in-home dialysis.

If you are eligible to stay in one of these shelters, you must bring a person with you who will assist you during your stay.

If you feel that you will need to go to a Special Medical Needs shelter during or after an emergency or disaster, you must fill out a form and register with your county management center PRIOR TO THE EMERGENCY. Now would be a good time to do that.

You can click on the link below, which will take you to your county emergency management website, where you can acquire the form that you must fill out. Or, you can call the emergency management center and request the form be mailed to you. Fill it out and mail it back to the correct address below. If you have questions, you can contact emergency management personnel at the phone number listed below for your county. Now would be a good time to ask those questions.

We train for disasters. The real  disaster would be losing a neighbor, friend or loved one because you had not taken advantage of that training.
