Forms for ALL Teams – due with registration

Chaperone form – Required to be signed and carried by all chaperones – at least one per team.

2010 Team Registration/Summary – One per team required

2010 Individual Registration form – 3 pages, first two required for ALL participants, last page is information only

2010 Player Medical Release form – required for all players

In addition to the above forms, all participants must provide proof of 24/7 insurance coverage good in the US (a photo copy of the front of their Health Sante card meets this requirement)

In addition to the above forms, all players must provide a government issued photo ID stating their birth date – If their Health Sante card has their photo and age, it will also meet this requirement.

In addition to the above forms, each team must have their volleyball governing body email us a letter of good standing. This letter must list the name of EVERY participant and state that they are all in good standing as both individuals and as a team.
