Program
Philosophy
The major objective of this program is to
maintain the clients' safety and enable him/her to live in
dignity.

Services We Provide
Psychiatric,
Psychological and Social Work intervention upon receipt
of the Probate Court appointment as guardians,
conservator or third party payee.

Secure
appropriate housing and ensure the provision of
appropriate care, comfort and maintenance of the client.

Assist
the client to receive the needed mental, physical and
support services needed.

Petition
the probate court to change legal status when needed as
the clients' condition changes.

Act as
an advocate on behalf to the client on applying for the
benefits which he/she is entitled.

Frequent
visits to the client on as needed basis. A minimum of one
visit monthly.

Maintain
contact with the family and other agencies involved in
the clients' situation as needed.

Arrange
transportation whenever needed to help the client keep
his/her medical and or psychological appointments.
-
Assume
responsibility for financial transactions with separate
accounts for each client.

Secure
placement in appropriate licensed facility whenever the
need arises.

Consent
to medical and mental care when needed.
-
Complete
the required Probate Court reports timely and accurately.

Make the
best effort to protect the clients' interests and legal
rights.

Quarterly
review of the clients' living arrangements.

Assessment
The social worker conducts an on site
assessment by interviewing the client as well as the family to
gather detailed information to enable the team members to define
the patients needs. A fully licensed Psychologist administers
Psychological evaluations, when needed.
Service Plan
The professional team with the family
input, develops an action plan tailored to the patients specific
needs.Our agency executes the plan utilizing the patients'
personal and financial assets as well as the public and private
sectors resources available.
