Process for applying for insurance coverage

1. Email Cameo at cameo@cameocare.com  with the following information:
   1. Your First and Last Name
   2. Daytime contact number
   3. Plan that you have Insurance through
   4. Problem you are having that requires Massage Therapy
   5. If car or work accident: please specify the company who will be paying your claims
   6. Claims which are in/or are going to be in litigation, will only be accepted if insured pays at time of service.
2. Cameo will then contact you within 72 hours and get the rest of the information she needs from you.   
3. She will then contact your insurance company or company handling the claims and verify your eligibility and the referral process needed to receive massage benefits.
4. Then she will call you with this information and then schedule an appointment or refer you to the next step needed in continuing the process acquiring eligibility.
