The following forms are provided in your Den leader's guide, or you can get them online using the links below.

Medical Forms

BSA has come out with a new medical form. Pack are still permitted to use the old medical forms, but it is preferred that you use the new ones. Note that there is not a separate form nor a requirement for a yearly exam under the new forms. All youth and adults will fill out parts A&C only. The new form does require a copy of the insurance card.

Den Roster

You should have a den roster for each den at camp (if your pack is bringing 20 boys and is being divided into 2 dens, then divide the boys up onto 2 Den Rosters). You will hand a copy of the den roster in at the registration desk. It may make check-in each morning easier if you have a copy of the roster in your notebook so that you can just verify that everyone on your roster is with you.
