The form comes in two formats: .PDF and .JPG. The .PDF is highly recommended. When you print the .PDF, make sure that "Page Scale" is set to "Fit to Printer Margins". To use the .JPG, you can save the .JPG file to your computer and place it into your favorite text document program (ie Microsoft® Word).
