Initiation Announcement

List the names of newly initiated members and send one copy to the Province President and one copy to the Executive Secretary. If this form is not received by the Executive Secretary, the National Fraternity will not know that candidates are initiated members. Membership certificates will be mailed after the Executive Office receives this form.

The Executive Secretary will assign the National Numbers on the form. The Executive Secretary will send one completed copy to the Chapter to be kept in the permanent Collegiate Member File. A copy also will be sent to the Province President for the Province files. Use additional sheets if needed.
