Star of D.O. Award

The chapter may award the Star of Delta Omicron to a member in recognition of outstanding service to the Fraternity, school, or community. The Star is a small charm that may be attached to the D.O. badge guard chain. The chapter assumes the cost. This form must be completed and received by the Executive Secretary before the Star can be sent.

Attach this form to the Chapter Payment IB Form IB 364. Enclose a check or money order payable to DELTA OMICRON FRATERNITY for the total number of Stars of D.O. ordered. Mail these to the Executive Secretary, 910 Church Street, Box 752, Jefferson City, TN 37760 or pay by credit card by contacting the Executive Office. Please allow 2 weeks time for processing. Send one copy to the Province President, and retain one copy for your records. Current price of the Star of D.O. may be found in the list of jewelry prices in fall supplies.
