Immediately following the Pledge
Ceremony, the 1st Vice President should list the names of
prospective members and send this form to the Executive Secretary along with a
prospective member fee ($20.00) for each person. Attach this form to Chapter Payment Form IB 364 before mailing.
A
copy will be forwarded to the Province President who will retain this list as
the Province Membership Record.
