2009 Background Check Procedures

 

NEW VOLUNTEERS:

1.       Volunteers can call 630-3510, 630-4146, 630-3573 or 630-3506 to schedule an appointment to be finger printed. Only those volunteers listed on a National Roster and/or on an Association’s Board of Directors need to have a background check completed.

2.      Finger prints will be done only by appointment; walk-ups will be turned away at our mass sessions.

3.      During the busiest times of the sport seasons; February-March for baseball/Softball and July-August for Football/Cheer, limited day time appointments will be available. Months other than these we will schedule appointments daily (M-F), on the half hour from 9am-11am and from 2pm to 4pm.

4.      Volunteer needs to bring their FL DL or ID with them to their appointment and they will at that time fill out a FDLE waiver form.

5.      Volunteer will be digitally printed and have their ID picture taken.

6.      Once we receive the results of the volunteer’s background, typically 12 to 24 business hours,  we will print and release the badge if the results are favorable and notify the association that they have badges ready for pick-up.  Badges will not be issued to individuals, only to association appointed representatives.

 

RETURNING VOLUNTEERS:

Returning volunteers will be defined as those volunteers that have a Department issued badge for the previous year in any sport.

Note: Please turn in the completed forms to the league president.

1.       Volunteers will fill out a Statement of Continuous Service Form (For a PDF of the form click here)

A.     Please fill in the section covering “Last Badge”, top left corner, by listing the sport your previous badge was for.

B.     Pop Warner Football/Cheer Only- Please fill out the section covering “ New Badge”, top right corner, to include Team name and Division of play.

C.     Please fill in “Name of Organization’ with your association.

D.    Please fill in “ Last name, First name” with your legal name, if the name on your last badge is different than your legal name then list that next to “First Name”, i.e. William  (Bill)

E.     Please complete the following; Race, Sex, Date of Birth, Social Security Number, address, home and work phone.

F.     Sign and date the document and give the form to an association representative who will witness the form with their signature.

2.      From this point the association will be responsible for bringing the CS forms to the RCSD office for processing.

3.      Once RCSD has received the CS form we will verify that the volunteer is eligible as a returning volunteer, document that of the CS form and then we will take the form to the ID software and create the new badge from your old image on last year’s badge. Once the new badge has been created it will be printed and the association will be notified and they will be responsible for picking them up and getting them back to the volunteers.  We will not process volunteers who walk into the office with their form, we will refer them back to the association.

4.      If for some reason we are unable to locate your picture in our ID software we will put “ Needs Pic” in the upper right hand corner of the CS form and return it to the association and ask them to give it directly to the volunteer listed on the CS form.  That volunteer can come down to the office with the form in hand and we will take a new picture of them and issue them their new badge.
