Registration and payment information for all 2009 summer camp programs.

All campers must complete all 3 steps on this page before participating in any camp activities:
1) register, 2) pay, and 3) complete the required medical forms.

1. Register for the Camp:

Send your registration/payment form and check to:

2. Pay for the Camp:

Print a Payment Form to mail with your check. Format: pdf or Word
(please write the player's name on all payments to ensure proper credit)

Pay for camp with your Credit Card by selecting your type of payment and clicking "Pay Now":

3. Complete Medical Forms:

This 2-page form must be mailed in or brought to camp with a copy of the camper's current immunization record and a Dr's signature if the camper is to be administered any medications at camp. Please download, print, and sign the form.
