2008 REGISTRATION FORMS

A 2008 HYSA Recreational Program Registration Form must be completed for all new and returning players.

Registration for the Fall 2008 Recreational season is now OPEN.

Registrations received after June 30, 2008 will be charged a $10 late fee per player. Absolutely no registrations will be accepted after July 31, 2008.

Completed forms can be delivered to any HYSA Board member, or mailed to:
