$50 per player  (Lunch  included)               Captains Choice:  4-player teams

4-player Team Sponsorship:  $260 (includes hole sponsorship)       Mulligans will be available for purchase! 

   Door Prizes Awarded immediately following tournament - 1st and 2nd place prizes, longest drive, closest to pin   

Entries should be received by April 24, 2009 to guarantee entry
