NOTE: when filling out the form you must use a black (or any dark color something like dark dark green blue what ever just as long as its dark) felt tip pen! that is very important so the font is big enough. also when making your little letters try not to draw the lowercase letters as big as you capitol letters. then it turns out really big and it makes for more work on me to make them smaller!
