Disability - Nathan B. Conrad
Before me Jeremiah Rex clerk of the Court of Common Pleas of Seneca County Ohio, a court of record, personally appeared Nathan B. Conrad, who being by me duly sworn according to law ----- and says that he is the identical Nathan B. Conrad who on the 3rd day of September AD 1862 enlisted in and became a member of Co. D of the One Hundred and Twenty Third Regiment of Ohio Volunteer Infantry, in the late war for the suppression of the southern rebellion. That he was honorably discharged from said service on the 28th day of June AD 1862. That at and prior to the time of his said enlistment in said service, in said company and regiment, he was and had always been a sound and able bodied man, capable of then doing continuously daily manual labor, in his occupation of a farmer and laborer, that was then able to perform all kinds of manual labor, without any inconvenience, that he was then sound, that he was not then affected with any disease of the heart, that he had no occasion of and had never called upon physician to prescribe for him on account of any sickness or disease. That about the 1st day of December AD 1864, while this declarant was on duty in said company and regiment, in the Shenandoah Valley in the state of Virginia, through hard marches and exposure this declarant was taken sick, became unconscious of where he was or what was going on around him, that he did not regain his consciousness until he found himself in a field hospital near Winchester, Va, that he was then suffering with a severe attack of Typhoid Fever, had a dull heavy pain in his left side in the region of his heart and extending down along his side- that he could not state how long he remained at the said hospital at Winchester- that he was then taken to a hospital at Baltimore but cannot now state the name of the hospital, that he was there severely blistered on his left side from the region of his heart downward, that his mid left side pained him so that he was unable to lay upon it, or allow any weight to rest thereon, that affiant became short breathed, his side and heart felt as though a great weight was at all times upon it- that he was then removed to a hospital at Little York, Pennsylvania- on the road(?) from Baltimore to this last named place, declarant took cold and again grew ____ at the last named place to suffer greatly in his - left side, that the least exertion would cause affiant to choke up, get short breathed, that he remained at Little York until some time about the tenth of March, 1865, when though very weak and feeble and suffering with the pain in side and his shortness of breath, declarant was sent to the Regiment to make room for other wounded soldiers. That by and through his said sickness and exposure upon marches, his heart became affected and this affiant says that he has never felt as he did before such sickness and exposure, that upon his arrival at his - regiment he found himself weak and feeble, easily put out of breath- suffering with pain in his side in the region of his heart, which became greatly aggravated by any eposure or any exertion in the performance of any duty- that shortly after his return to the army peace has --- and he continued with Company until its final discharge in June 1865, that he was not able to and did not perform any but the lightest duty That upon his return home, he was unable to perform manual labor, and his condition continued to grow worse until he was compelled to and did employ Dr. E. W. Reeves of Tiffin, Ohio to treat his case, and that he continued from time to time under -- Dr Reeves care and treatment until sometine in the May of 1870, when -- Dr. Reeves died at Tiffin, Ohio, that since that time he has been under the treatment of Dr A. B. Hosey(?), of Tiffin Ohio, whose affidavit he will attach to this declaration, that -Dr. Reeves as soon as he commenced the treatment of affiant informed affiant that affiant's heart was affected - that affiant's physical condition continued to grow worse until affiant is not now able to perform any kind of manual labor, that he did not employ any other physician but Dr. Reeves during his (Reeves) lifetime and he is therefore unable to supply medical testimony after that point., that he was repeatedly urged to apply for a pension but affiant refused hoping that his disease would improve, and he would not be a burden to the government. That from the disease so contracted in the said military service of the United States- this affiant is now unable to perform manual labor, that upon the least exertion he has shortness of breath, palpitation of the heart, and he is unable some days attend to the easiest chores, that the change in the weather affects affaint - some days he is up as others in his bed- that he is therefore compelled to ask that his name may be on the pension rolls of the United States by reason of his disability. That since his return form the army he has be employed in his usual occupation that of a farmer, but has been unable to perform his work and has been compelled to hire help to accomplish his work- and now is compelled to abandon it altogether, That since his discharge he has resided in and near Tiffin, Ohio, that his post office address is Tiffin, Ohio, that he never performed any further military service than above stated. That he hereby appoints A. L. Brewer of Tiffin, Ohio his attorney to prosecute this claim, and do whatever may be necessary to effect the object of this appointment. Nathan B. Conrad. At such time and place came also Walter Snyder & Edward Jones, both of Tiffin, Ohio, who being by me duly sworn according to law that they reside in Tiffin, Seneca County, Ohio, that they are personally acquainted [with] Nathan B. Conrad who subscribed the _____.declaration in their presence that . Nathan B. Conrad was a member of Co. D, of the 123rd Regiment of Ohio Volunteer Infantry in the late war for the suppression of the southern rebellion, that from their acquaintance with him they know that he is the identical person he represents himself to be, that they were present ____ ___ this Nathan B. Conrad sign his name to the ____ declaration and that they have no interest in his claim for a pension. Walter Snyder Subscribed in my presence and sworn to before me this 18th day of December AD 1876 by the said Nathan B. Conrad, the said Walter Snyder and said Edward Jones and I hereby certify that said Nathan B. Conrad is the identical person he represents himself to be, that said Walter Snyder & Edward Jones are credible persons and all are personally know to me, that I have no interest whatever in the persecution of this claim for pension, that the above erasure was made necessary by mistake in affixing signature.
