To schedule an appointment, please call during our offices hours. In consideration of our other patients, please make every effort to be on time, as arriving late may require us to reschedule your appointment. For new patients, please arrive early to fill out the necessary paperwork. To save time, you may download and complete the Patient Information form found in the Forms section of this site. |

| We offer many payment options. Besides accepting the major credit cards, we offer low and no interest financing options. |

| Insurance |
| |
We accept most dental insurance plans and we are always happy to help you file your insurance claims. Please notify our office of any changes to your policy as soon as possible, since our speed and efficiency is only as good as the information you give us.
You will also need to be prepared to pay any amount that is determined not payable by your insurance plan, such as deductibles and percentages as payment. Such payments are due at the time service is rendered. |
