The purpose of this Permission
to Create and Use Photograph of Performance form
("Permission Form") is to record your permission to
create and use any photograph(s) of your performance at
the stated venue, and to publish it (them) as part of
LucidMusic.com, an online music photography gallery owned
and operated by Roger Miller, dba Lucid Music, LLC. Upon
request, links to published images, or small-format
web-quality files, will be made available to you for
online promotional use.
If this is acceptable, please
fill out and sign the electronic form below.
Alternatively, please return a paper copy of the
printable
form in person, either
to Roger L Miller or to a management member of the
performance venue, anytime up to the start of your show.
All Permission Forms must be
received prior to any photograph(s) being included in any
online gallery or used in promotions.
By signing below, you grant to
Lucid Music, LLC, its successors and assigns, the
non-exclusive right and license to create, use,
reproduce, display, and distribute, but not offer for
public sale, your photograph on LucidMusic.com and the
performance venue's website, and in advertising and
promotional reproductions in any media or format, whether
now or hereafter known, including without limitation to
other kinds of print media and electronic media (such as
on-line systems, CD ROM applications, and multimedia
products). The copyright of your image will remain the
property of the submitting photographer, whose permission
must be obtained for any additional uses outside of this
agreement.
Your signature below also grants
to Lucid Music, LLC permission to use your performance
name or group name and website address supplied on the
Permission Form should Lucid Music, LLC, in its sole
discretion, decide to do so. It is expressly understood,
however, that Lucid Music, LLC is under no obligation
whatsoever to include your image(s) or any of this
information on LucidMusic.com or the performance venue's
website.
