Instructions:

  2. When you are done making selections, choose your shipping location.
  3. When you are finished making your selections, click on the "Calculate Totals" button.
  4. If you want to make any changes, click on the "Clear Form" button and start over.
  5. Enter the address you want to use for shipping.
  6. When you are ready to print the order form, click on the "Print This Order" button. A new page will appear with a summary of your order.
  7. Print two copies of the new order form. Keep one and send the other with your payment in U.S. funds.
  8. Please allow four weeks for US deliveries, six weeks for international orders.
