Financial Commitment Agreement

Fall/Winter Fee Schedule

_____ Option I: Payment of total fees due in full at registration. (High School Swimmer must chose Option I)

_____ Option II: Payment of 50% of total fees due at registration with the remaining 50% due by January 10th, 2006.

_____ Option III: Payment of 1/3 of total fees due at registration with subsequent monthly payment of 1/3 fees due by November

1st, 2005 and January 10th, 2005.

All USS Swimming cards must be paid in full at time of Registration.

For options II and III, if fees are not paid by the 10th of the month a late fee of $20 will be assessed.

* If a family's dues are in arrears for a period of 1 month (30 days) and the family is currently being billed for a second month, their swimmer will not be allowed to participate in the program until their account balance, including any penalties/fines, is zero.

 

The total fees must be paid even if a swimmer ceases to participate with the Piranha program, either temporarily or permanently (for illness, vacation or any other reason). Payment should continue to be made as selected in the Payment Option section of this Agreement. If a situation arises that you feel entitles you to terminate this Agreement without paying the total dues you must contact either Julie Kawalski or Jacquie Quinlan. Your situation will be considered, evaluated and a determination will be made. A swimmer is not considered withdrawn from the program until written notice of the withdrawal is received by the Piranha Team.
