Appointments are scheduled on the hour, and first new admission appointments are asked to arrive 15 minutes before their appointment hour to complete basic admission forms. A clipboard with the appropriate forms for your specific referral requirements will be left in the reception Room, with a card, with the first name identifying the person to be treated. We ask that you bring your Insurance card for us to copy, and determine if any special information that your benefit program requires, including any "Authorization numbers" provided by your Insurance or Managed Care Organization, or your EAP, if so required. All sessions are scheduled for 45-50 minutes, but may require a longer time, and may be adjusted if time and schedule permit. |
