Fee Schedule

All fees were set by my Partnership Group in 1994.  These fees are standard for all the members of the group, and they conform to "UCR" (Usual, customary and reasonable) for our region and levels of practice.  Many fees are adjusted and controlled by agreements and contracts with various Insurance managed care organizations and Employee Assistance Programs  Co-payments may apply according to your insurance, or managed care company, or EAP  They must be paid at the time of service, unless other arrangements have been made.

In cases of demonstrated need, fees may be adjusted due to your special circumstances.  Call for information if you feel this may apply to you.
