Once you have filled this out and sent it back to us via e-mail I will call you at the phone number you provided to tell you if your application looks good and to set-up a time for you to come and meet the puppies! You can also print it out and snail mail it back to us at the address at the top of the application...if you want to keep your info off the inter-net although our e-mail is secure so no need to worry!
