2009 Season Registration

This year all registration will be by mail and in two parts.

Your initial registration must be postmarked between November 1, 2008, and December 6, 2008. This is just basic information that we need to start the season and requires no money. Click here to download the Registration Form. If your registration is postmarked after December 6, 2008, and there is still room on the tryout roster, and if your late registration is accepted, a $25 fee will be added to the cost of your registration.

Your registration Fee of $150 (for the first installment of Team Dues, payable to "Southridge Booster Council"), your Medical Waiver (page 6 of the Registration Form or the Southridge Sports Medical, either current within 2 years prior to June 30, 2009) and your US Lacrosse Membership confirmation (membership must be current through July 31, 2009), must be mailed and postmarked no later than January 9, 2009. Incomplete Registration or Medical forms will be returned. Please make sure to read and sign everything.

The remainder of the dues must be paid when Uniforms are issued (February 28, 2009).

Registrations postmarked after January 9, 2009 may not be considered for placement on the tryout roster and your registration and fee will be returned to you, if the tryout roster is full. We are looking at placing no more than 80 candidates on the tryout roster for no more than 65 player positions. If your registration is postmarked after January 9, 2009 and there is room on the tryout roster and if your late registration is accepted, a $25 fee will be added to the cost of your registration. This is no guarantee that any late registration will be accepted.

Mail all forms to Coach Campbell (address on registration form). Please follow the form instructions carefully. Late registrations will be returned. Remember that we will have 2 Teams: Varsity & JV and we may have a JV2 Team if we have sufficient Coaching Staff and enough players. Each team is limited to 20 to 25 players. Tryouts rosters are also limited, so a late registration may result in your inability to participate in the tryouts.

Registration Form
Due by December 6, 2008:
Team Registration. You can download the Registration form (in Adobe Acrobat format) by clicking here: Registration Form. The form requires Player and Parent signatures. Players 18 or older do not need Parent signatures. There are seven (8) parts of the Registration Form, as follows:
• Player Biographical Information;
• Participation Form;
• Release of Liability;
• Transportation Release;
• Website Permission;
Code of Conduct Agreement.
• Medical Release;
• US Lacrosse Membership Online Confirmation.

The reason for the earlier Team Registration is to gauge number of players we may have and to forecast the amount of certain types of equipment we will need for the Season and so we can make Coach assignments and practice field allocations. If you do NOT Register we cannot guarantee that there will be enough Team equipment for you to use. Completing and sending this form to Coach Campbell will ensure that you are included in future e-mailings regarding actual Team activities, Website updates and other important Team ramp-up information.

Due no later than January 9, 2009:
• Medical Release;
• US Lacrosse Membership Online Confirmation (a copy; keep the original for you records).
• An initial $150 Dues payment must accompany these forms. When the Teams are selected, after tryouts, you will pay the remainder of your Dues. Click Dues for more information on what is required.

Please pay careful attention to the following:
• If the Player has a "Medical" form on file with the Southridge Athletic Department the Player must obtain a copy of that form and submit it during registration. Coaches do NOT have access to school files and will not request or obtain the school Medical form on the Player's behalf. Failure to submit the form will cause the Player to be removed from the Team Roster;
ALL players must submit a Medical Release Form:
• ALL PLAYERS, New and Returning, must submit a Medical Release that is current within 2 years prior to June 30, 2009 and MUST have the Medical Release Form signed by a Doctor. Remember to schedule a Medical Exam early enough so that you will be able to submit this portion of the Registration Forms on time;
• Additionally, ALL players must provide accurate and current information concerning their Health/Medical Insurance coverage. This information is a required portion of the Registration form (Participation section). If a player does not have personal/family Health/Medical Insurance the player must purchase insurance through the School District in order to be eligible to participate in practices, games and/or other Team activities;
• For the 2009 season ALL players must be current US Lacrosse Members through July 31, 2009. This is to ensure that all players are eligible for all in-season games & practices (from February 16, 2009) through the post season (as far out as July 31, 2009). Remember to include a copy of your US Lacrosse Membership Confirmation receipt with the rest of your Registration paperwork. To apply for US Lacrosse Membership go to the US Lacrosse Membership website for details on how to apply and to obtain information on the benefits of membership. We require ALL Players to be current members of US Lacrosse, which is the national governing body for all levels of Lacrosse. Aside from other membership advantages, US Lacrosse provides major medical insurance coverage for all US Lacrosse sanctioned events, which includes tournaments, games and practices. Players MUST be current US Lacrosse Members to participate;

• Make sure you apply for US Lacrosse membership as a HIGH SCHOOL player;
The name of the Team is "SOUTHRIDGE SKYHAWKS". Include this in the section where the form requests "What is the name of your team?" Failure to do this will exclude from the US Lacrosse Roster that we will use for games & tournaments.

If this fee is not included your registration will be returned. No registrations will be accepted after January 16, 2009, even if the tryout roster is not filled. Failure to submit ALL parts of the Registration Form, completed and fully signed, will void your registration and it will be returned to you.
