LICENSEE COMPLAINTS NOTE: If you desire to file a complaint against a licensee/provider then you may use the following email addresses to file that complaint. When filing a complaint remember to be specific but concise. Include who was involved, what occurred, when it occurred, where it occurred, why it occurred, and how it occurred. List all witnesses, including their contact information. Always keep a copy of the complaint and follow-up with the complaint if you do not receive a response within 7 days. State facts and clear conclusions. Do not ramble on. Only include pertinent information. If you can, briefly state the law or ethics violation that the licensee has violated. If you have any evidence or documentation, make a copy and attach it to the complaint. If you need help with filing the complaint, you may call the licensing board and they will be willing to assist you. Remember, the board is on your side. Understand that sometimes the licensing board may disagree with your complaint. Do not be discouraged. If you need any further assistance, you can always email P.R.A.C.
