A welder sustained burns to their face and hands at a fabrication shop during a production operation. The welder was pre-heating a nozzle on a vessel head and noticed they were not getting the normal flame from the tiger torch and could smell propane.

123 CSB Safety Bulletin: Dangers of Working on Piping and Equipment That May Contain Flammable or Chemically Reactive Materials The Bulletin recommends that facilities should: physically examine all piping and components between isolation devices such as valves, and be sure piping drawings are current; use the drawings to identify key components, such as low-point drains that can be used to remove dangerous chemicals; prepare a specific written procedure for removing hazardous material and consider the consequences of working on piping that is not completely purged.

148 Accident Prevention Tip: Barricade Dangerous Areas It has become common practice to use outside contractors for mine and plant maintenance projects on an as needed basis. While this approach is wide spread in all industrial settings, it brings some additional risks in the unique area of mining. Recently a fatality occurred at a cement operation when a contract worker wandered away from his work place inside a large transfer duct. The victim walked on the crusted surface of a mound of hot dust. When the material unexpectedly gave way the victim fell waist deep into the hot dust and suffered fatal burns.

201 CSB Case Study: Oil Refinery Fire And Explosion CSB NO. 2004-08-I-NM, October 2005. This case study describes the sudden release of flammable liquid and subsequent fire and explosion that occurred on April 8, 2004 at the Giant Industries’ Ciniza oil refinery, Jamestown, NM. The incident injured six employees and caused evacuation of non-essential employees as well as customers of a nearby travel center and truck stop. Refinery equipment and support structures were damaged. Production at the unit was not resumed until the fourth quarter of 2004, and damage to the unit was in excess of $13 million. Because of the serious nature of the incident—injuries to employees and extensive damages to facilities—the U.S. Chemical Safety and Hazard Investigation Board (CSB) produced this Case Study to share lessons learned so that similar occurrences might be prevented.

The investigation into the fire and explosion at the Buncefield oil terminal depot on 11 December 2005 is continuing. Enquiries into the workings of the high level safety system to prevent tank overfill have indicated that certain aspects of its installation and testing are critical in ensuring that it works properly. Operators of similar establishments are therefore advised to carry out immediate checks where TAV level switches[1] manufactured by Cynergy3 Components Limited and supplied by them, their predecessor companies[2], or others, are fitted to their storage tanks.
