It appears the form submittal services that were being used to support this site are no longer working. The form service sites are not responsive to inquiries about their services. Therefore, the input forms for this site have been closed down until another service that will successfully deliver the information submitted is found.
This was only discovered after form mailings stop abruptly. I have no way of knowning how many submittals were not delivered.
