undersigned apply to the United States Orienteering Federation (USOF) for
issuance of a charter for the formation of a sanctioned regular club and apply
to affiliate with the United States Orienteering Federation.
Five
or more persons who are USOF members may form a regular club and receive a
charter. If a club does not have five
USOF members, personal membership applications may be submitted with this
application or the club may apply for an associate club charter.

Annual
associate club renewal fees are payable thereafter by February 1 of each
