On Jason and Courtney's wedding day, Carly is kidnapped by Ric right in front of Michael. Ric takes her to his and Elizabeth's new home, and holds her in a "panic room." Believing that Sonny was responsible for Elizabeth miscarrying their child, Ric is determined to hold Carly until she gives birth; take the baby; and then kill her. Sonny believes his new business enemy, Lorenzo Alcazar (the brother of the deceased Luis Alcazar) is behind the kidnapping. Eventually, Alcazar does get involved, and take Carly from Ric to use as leverage with Sonny. Alcazar wants to move drugs through Sonny's territory. After trying everything else, including cooperating with the police and FBI, Sonny, with Jason & Ric, orchestrates a rescue in South America, and Carly is finally brought home. But Alcazar decides that he wants Sonny's wife more than he wants to move his drugs, and he begins to stalk her and send her presents. Sonny senses that Carly has some feelings of her own for his enemy.

Alcazar will stop at nothing to get Carly away from Sonny. He pays a hooker to have plastic surgery, so that she looks just like Sonny's dead wife Lily Rivera. "Lily" begins haunting Sonny, and tells him that as long as Carly & their unborn child are with him, they are in danger. The sick plan works, and Sonny, who is losing control of his mind and his life, tells Carly they can't be together anymore. He buys her a house to live in. She goes there; falls down the stairs, and goes into labor. Of course, Alcazar is the one who finds her there, and helps her as she delivers the baby. Sonny arrives at the house, and hearing his wife screams, busts in the door with his gun drawn. Seeing his enemy leaning over his distraught wife, Sonny shoots Alcazar. Unfortunately, the bullet goes through him...and into Carly's head. She delivers Sonny's son, and then lapses into a coma. While in a coma, Carly dreams of an alternative life where Alcazar is the man she loves, and Sonny is the bad guy. When she wakes up, she has no feelings for Sonny or her children...but plenty for her stalker. When Sonny realizes this, he goes to kill Alcazar. But Carly calls and warns him, and Sonny walks right into a police ambush. Devastated and desperate, he takes a hostage and escapes. The hostage is Samantha McCall; a young and beautiful woman involved with Jasper Jax. Eventually, Sonny gives himself up to the police, and is put on trail for attempted murder. He is acquitted. Carly then "regains" her feelings for her husband...but also retains her feelings for Alcazar. When Sonny leaves on a business trip, Carly almost sleeps with his enemy. Sonny gets detoured on his trip, when Sam McCall rushes onto his plane. She needs his help getting away with a "found" treasure...a man is after her. Sonny helps her, and they form a friendship while stranded together on an island. Sonny returns from his trip, to find his wife in Alcazar's arms. He says their marriage is over...and when he runs into Sam on the docks later that night, there's nothing to stop him from falling into bed with her.

Carly finds Sonny in bed with Sam, and immediately jumps into bed with Alcazar. Sonny believes Carly isn't fit to have their children with her, and a nasty custody battle ensues. Wanting to help his new love, Alcazar plants a bomb in Sonny's limo...which explodes outside the courthouse during the hearing. Sonny decides he can't keep his children safe and gives up custody to Carly. Sonny & Sam continue to spend time together, and she becomes pregnant with Sonny's child. When Carly discovers that Alcazar set the car bomb, she leaves him...even though it was no worse than anything he'd done to Sonny before. She takes the boys back to Sonny's, and he finds himself stuck between wanting to give his sons their family back, and his new relationship with Sam. Sam keeps her pregnancy a secret from Sonny for quite a while, but eventually he finds out the truth. Jax believes the baby is his, and Jason takes it upon himself to doctor the paternity to say that HE, not Sonny or Jax, is the father. Sonny reluctantly goes along with the ruse...which he finds out about after the fact...so that his family with Carly can remain intact. Sam's ex-boyfriend comes to town, working for Faith Roscoe who still wants Sonny dead. He shoots Sonny, who calls out for Carly. They reunite because of the shooting, and Sam continues to live with Jason. Sonny tells Carly the truth about the paternity of Sam's baby, and they remain together. However, Carly's holding onto a paternity secret of her own...that Sonny is the father of Kristina Davis, Alexis' daughter. That secret will soon blow their lives apart once again.
