Registering with USA Hockey

USA Hockey membership is required for all players and coaches.  Players MUST register with USA Hockey at www.usahockeyregistration.com. Note that our local USA Hockey affiliate is the Potomac Valley Amateur Hockey Association. No player will be allowed to skate for the Club unless he/she is registered with USA Hockey.

Proof of membership will be provided by USA Hockey at the completion of the process.   Please forward the confirmation e-mail to the Ice Hockey Club or print a copy of the confirmation HTML page to give to a coach at practice.  The confirmation e-mail or page must include the player membership number and the associated bar code to be valid.

NOTE: If you are already registered with a select or travel team, you should contact that team's registrar and obtain your Player Previously Registered (PPR) code.

Player Fee Payment Instructions

The fee is $600.  (This is the same as last year's fee, but this year there will be a lot more hockey).  Please make a check payable to "Yorktown Ice Hockey Club".  You may give the check to a coach or mail to Yorktown Ice Hockey Club, 3520 N. Abingdon St., Arlington VA  22207.  The Club greatly appreciates prompt payment, as our highest cost item (ice slots at Ballston) is due to Kettler IcePlex now.

All Forms Required to Complete Membership

All forms are to be submitted to the Club by December 1, 2009. USA Hockey requires each team to maintain the executed forms. Please fill out and sign all required forms and either mail to the Yorktown Ice Hockey Club at the address above OR give to a coach at practice.
