Before I begin, I want to point out that I don't consider the use of "An Unearthly Child" and the like instead of their recently discovered "proper" titles to be a mistake. I also don't consider the frequent use of the term "Doctor Who" in reference to the Doctor in the descriptions to be a mistake.
An Unearthly Child
The description refers to the time the cavemen live in as the Great
Cold. In the show, the Great Cold merely referred to the coming of winter.
The Daleks
Thals is misspelled Thais.
The Aztecs
The quote says, "You cannot rewrite history, not one line." I'm fairly
certain the Doctor used the word "can't" in the actual show. In the two
photographs on the back, William Hartnell is out of costume. (rehearsal
photos)
The Dalek Invasion of Earth
The year the story takes place in was not 2164 as the cover says it
was (this was just the date on the calendar that Ian finds, but it's years
after that calendar was made) The Earth is not the only planet with
a magnetic core. The TV story doesn't
claim that it is.
The Rescue / The Romans
The rescue ship was not intent on revenge on the Didonians. Time
was not running out for the planet. How exactly can a person spend
the rest of their life trapped in 64 AD (unless they die before the end
of the year, of course)?
The Web Planet
The TARDIS crew "stray from their astral plane". Is Shirley McClaine
piloting?
The Crusade / The Space Museum Collector's Edition
On the top of the back side of the cover, Glyn Jones' name is misspelled
Gyln Jones.
The box claims to contain "four high quality collectible images from
The
Crusade" on the postcards. Actually,
only three of the images are from The Crusade. The fourth
is from The Space Museum.
There's a missing dash in the description of The Crusade when
it says "It's the 12th Century Palestine and King Richard..."
The box is misleading when it says it is in stereo. Only the
title logo sounder is in stereo. The episodes are mono.
The Daleks Limited Edition Boxed Set
Dalek is used singularly in one instance where it should be plural.
The box says The Chase was Produced by Terry Nation. It should be
Verity Lambert.
The Ark
The box says the story was Produced by Verity Lambert. It should
be John Wiles. Also, Lesley Scott's name is misspelled
Leslie Scott in some places, but correctly as Lesley Scott in others.
The box also says that it is in stereo. While this is true of the
music that accompanies the formation of the video logos, it isn't the case
on the program itself, so I'd call this a mistake too.

Alistair Pearson's name is spelled Alister Pearson.

While this may be what actually happened, he didn't get the credit on screen
and thus this breaks the normal pattern of the videos where they normally
use the pseudonyms and names that appeared on screen as opposed to the
"real" names that did it.
The War Games
On the top of the box, it says "The War Games Part 1." Nowhere on the
top does it say Part 2 is in there too (which it is).
Spearhead From Space
The box refers to "an evil alien" which should be plural. The box also
says that the Doctor chose to spend his exile on Earth when in fact he
had no say in the matter.

The box says the Doctor is called to Earth for this adventure as though
he'd been away just before this.
