SURVIVOR: OKINAWA -- The Official Application

First off, there will be a very short application process for the Okinawa game. So if you are interested, please fill this out and email it to us as soon as possible. The application process will be open for no more than ten (10) days so if you want to play, don't put it off!

From this initial application approximately 30-50 Semi-Finalists will be selected who will undergo intensive physical and psychological training in LA before 18-20 are then chosen to travel to the beautiful islands of Okinawa for Green Beret training. After the training period, 8 men and 8 women will be selected to embark on the adventure of a lifetime off the coast of Japan... for Survivor: Okinawa.

Please note the following eligibility requirements must be met in order to proceed with the application process.

a. You must be willing to participate in Psychological/Physical training either in Los Angeles (travel at your own expense) or via computer for two to three days if you are selected as a semi-finalist.
b. You must be willing to travel to a remote location in the Ryukyu island chain off the coast of Japan for up to ten (10) weeks in October, November, December and January, 2003-2004 (or as otherwise scheduled by the Producers). Economy travel to be paid for by the Producers.
c. You must be willing to participate in the game if you are selected (At least e-mailing once every other day.)
d. You must be at least 16 years of age (recommended age is 18 or over, but exceptional applicants aged 16 and 17 will be considered).
e. You must be in excellent physical and mental health.

Applications will only be considered if they are complete and TYPED. Complete applications consist of the following:

a. Completed application form
b. A photo. (optional, but recommended)
d. 500-word (or less) essay: Who are you and why you would make the ultimate SURVIVOR? Be creative!
