4. Parent/Gaurdian Name-This is the name of the parent or gaurdian of the person who wishes to be hired.

5. Emergency Phone Number-This is a phone number that The Green Thumbs will call if the home phone number is unavailable. This is critical in case something bad happens to you during our work.
