I-84 Westbound:
Get off at exit 48, Asylum Ave. Go right at end of ramp. Travel 1.5 miles on Asylum Ave to Scarborough Street. Turn right onto Scarborough St. Go to the end, and turn left onto Rte 44 west (Albany Ave). In several hundred yards, turn right onto Rte 189 (Bloomfield Ave). Go 1.2 miles, and turn left onto Abrahms Boulevard. Hebrew Health Care is at the end of Abrahms Boulevard.

I-84 Eastbound:
Get off at exit 43, Park Rd. Turn right at the end of the ramp. Make an immediate left onto Trout Brook Dr. Travel 2.5 miles to Albany Ave, Rte 44. Turn right and follow Albany Ave 1 mile to Route 189. Turn left onto Route 189 (Bloomfield Ave). Go 1.5 miles, and turn left onto Abrahms Boulevard. Hebrew Health Care is at the end of Abrahms Boulevard.

I-91 North or Southbound:
Get off at exit 35 A-B, Rte 218. Head west on Rte 218, (toward Bloomfield) and follow 218 for about 3.75 miles. Turn left onto Rte 189 (Bloomfield Ave). Go 1.25 miles, and turn right onto Abrahms Boulevard. Hebrew Health Care is at the end of Abrahms Boulevard.

Enter the main entrance; the MultiPurpose room just behind the receptionist desk to the right as you enter the lobby.
