original and need a place to keep a 'copy' :)

This is copied from a guide that was given to me by
my doctor. Since it has a company
name on it ( Eli Lilly and Company; Indianapolis, Indiana 46285), I assumed my
physician obtained it from them (I hope they don't mind my placing it here [especially
since I do not charge for it or its use]). I have it here mainly for my own use (in case I
loose the original or misplace it and can not locate it). I will have a copy here. I am
hoping others could benefit from it too. :)

some are free.

It is amazing what one can find online.
