Please note that we accept payment only from those insurers with whom we are participating providers.  All copays and excess charges are due at the time of service.  FOR ALL OTHER INSURERS WE REQUIRE PAYMENT IN FULL AT THE TIME OF SERVICE.  We will provide you with a super bill to submit to your insurer for reimbursement.
