    I would like to remind you of the district policy for medication administration at school.  Medications should be delivered to the school by a parent or responsible adult.  For safety reasons medication cannot be transported on the bus with students.
    Medicines should be delivered to the school in a pharmacy or manufacturer-labelled container with specific instructions for use.  Please ask your pharmacy for separate bottles for home and school.  No more than a thirty day supply of the medicine should be delivered to the school at a time.  There is a ten day limit for administering non-prescription medications.

    The following forms must be on file before we give any medicine at school.

1.  SIGNED CONSENT BY THE PARENT OR GUARDIAN to give the medicine at school.

2.  SIGNED MEDICATION ORDERThe written medication order form should be taken to your child's physician for completion and returned to school.  You doctor may also fax us the order at #529-9753.  This order must be renewed as needed and at the beginning of each school year.

    When your child needs a medicine to be given during the school day, please act quickly so we may begin to give the medicine as soon as possible.  Thank you for your help.
